Terms and Conditions

1. These Terms and Conditions apply to Phoenix Health Fund’s ‘Refer a Friend’ promotion. Instructions on how to enter and win prizes form part of these conditions of entry.
2. The Promoter is Phoenix Health Fund ABN 93 000 124 863 of 1/4 Honeysuckle Drive, Newcastle NSW 2300. Phone 1800 028 817.
3. Entry into this promotion is deemed acceptance of these Terms and Conditions. To the extent of any inconsistency between these Terms and Conditions and any other reference to this promotion, these Terms and Conditions will prevail.
4. The offer is open from 01 June 2022 until further notice. The new policy must have a commencement date on or after the promotion start date to be eligible.
5. To redeem this offer the New Member must email [email protected] with their new membership number (New Member) and the full name and membership number of the Phoenix Health member they were referred by (Referring Member) within forty-eight (48) hours of completion of their membership application.
6. The offer is available to current members of Phoenix Health (Referring Member), who refer Phoenix Health to new Members (New Member), and as a result of that referral, the New Member purchases a combined Hospital and Extras Cover.
7. A New Member is defined as a Member who has not previously been a Member of Phoenix Health, or who has not been a Member of Phoenix Health for at least six (6) months prior to re-joining the Fund. This includes any spouse/ partner or dependants on their policy.
8. A Referring Member must be a current Member of Phoenix Health and who has an existing policy with a status of ‘Financial’, that is, all premiums owing against the policy have been paid.
9. Subject to satisfying all criteria listed in these Terms and Conditions, both the Referring Member and New Member may each be eligible to receive a $150 Digital Gift Card (Gift Card).
10. A Gift Card will be emailed to the New Member approximately six (6) weeks after the application’s nominated policy commencement date, at which time the New member’s policy status must be deemed Financial.
11. A Gift Card will be email to the Referring Member approximately six (6) weeks after the commencement of the New Member’s policy, at which time both the New Member’s policy and the Referring Member’s policy must be deemed Financial.
12. Gift Cards will be sent to the Policy Holder on the Membership and delivered via email to the email address listed on the Membership. Phoenix Health is not liable for lost, damaged or stolen Gift Cards and will not replace any Gift Cards in the event of loss, damage, theft or expiry.
13. The New Member must not have cancelled a Phoenix Health policy within six (6) months prior to re-joining Phoenix Health.
14. ‘Refer a Friend’ promotion not valid with any other offer or promotion.
15. ‘Refer a Friend’ promotion must be redeemed within forty-eight (48) hours of new membership completion and is not available retrospectively.
16. Limit of one (1) Gift Card per New Member during their lifetime. Unlimited Gift cards are available to Referring Members who meet the eligibility requirements of this Promotion.
17. Phoenix Health reserves the right to deduct an amount equivalent to the value of the Gift Card from the New Member, should they cancel their policy within six (6) months of joining or re-joining.
18. Phoenix Health reserves the right to recover the value of the Gift Card should the New Member or Membership be suspended or cancelled by Phoenix Health within the first three (3) months of the commencement of the policy.
19. Phoenix Health have the right to amend these Terms and Conditions and may from time to time substitute or amend the reward for this promotion. Phoenix Health reserves the right to cancel this promotion at any time, without notice.
20. This promotion is only applicable to policies purchased directly from Phoenix Health. The Promotion is not available for policies purchased from third party sellers, aggregators or brands.

3. Entry into this promotion is deemed acceptance of these Terms and Conditions. To the extent of any inconsistency between these Terms and Conditions and any other reference to this competition, these Terms and Conditions will prevail.
4. The offer is open from 01 February 2025 and closes at 11.59pm (AEST) on 30 June 2025. Any applications received after 11.59pm on 30 June 2025 may not be eligible for the offer.
5. The offer is available to new members who join directly via the website, or over the phone and who purchase an eligible Combined Hospital and Extras or Hospital only policy within the promotional period.
6. A New Member is defined as a Member who has not previously been a Member of Phoenix Health, or who has not been a Member of Phoenix Health for at least six (6) months prior to re-joining the Fund. This includes any spouse/partner or dependants on their policy.
7. The offer is not open to current members of Phoenix Health. Any ex-Phoenix Health members (including spouse/partner or dependant) who re-join the Fund within six (6) months of the date of the cancellation of their prior policy, will not be eligible to claim this offer.
8. The promotion is not available to Phoenix Health Fund staff, immediate family, or Board Members.
9. The free month will be applied after a minimum of one month of premiums has been paid by the member (i.e., the second month of membership will be free).
10. Where a policy is paid fortnightly, two (2) free fortnights will be applied (i.e., to the 3rd and 4th fortnights).
11. Where a policy is cancelled within the thirty (30) day Cooling Off Period, no free month will be applied. Where a policy is cancelled within the period that has been applied for free, the policy will be terminated from the policies previous paid to date.
12. Limit of one free month in the lifetime of a member/ membership.

14. Phoenix Health reserves the right to deduct an amount equivalent to the value of the free month from the New Member, should they cancel their policy within six (6) months of joining or re-joining.
15. Phoenix Health reserves the right to recover the value of the free month should the New Member or Membership be suspended or cancelled by Phoenix Health within the first three (3) months of the commencement of the policy.
16. Phoenix Health have the right to amend these Terms and Conditions and may from time-to-time substitute or amend the reward for this promotion. Phoenix Health reserves the right to cancel this promotion at any time, without notice; or extend the promotion, at their discretion.

4. The competition is open from Tuesday 30 September 2025 and closes at 5.00pm (AEST) on Monday 20 October 2025. Any survey responses received after 5.00pm on 20 October 2025, or outside of this competition period will not be eligible for entrance into the draw to win.
5. Prize includes one of five digital Your Choice Gift Card valued at $200 which can be redeemed online from any of the stores available at truerewards.com.au
6. The competition is available to members who complete the Member Satisfaction Survey during the competition period and provide their member number on completion of the survey.
7. The competition is not available to Phoenix Health Fund staff, immediate family, or Board Members.
8. The ‘Member Satisfaction Survey October 2025’ competition is not valid in conjunction with any other offer or promotion.
9. To be eligible to win, Members must be active and financial at the time of winning. Phoenix Health reserves the right to recover the value of the Gift Card should the Member or Membership be suspended or cancelled within three (3) months of the end of the competition.
10. Winners will be drawn at random with an online generator on or before 20 November 2025 at IPSOS pty ltd, 6/11 Wilson Street, South Yarra VC 3141. Winners will be notified by email following the draw.
11. If any of the prizes are undeliverable or unclaimed 14 days following the draw date, as detailed above, no substitute prize or monetary recompense will be provided and a redraw will occur.
12. Prizes will be sent by Phoenix Health to the Policy Holder on the Membership and delivered via email to the address listed on the Membership. Phoenix Health is not liable for lost, damaged or stolen Gift Cards and will not replace any Gift Cards in the event of loss, damage, theft or expiry.
13. All prizes must be taken as offered and are not transferable, exchangeable or redeemable for physical Gift Cards or as cash.
14. Phoenix Health have the right to amend these Terms and Conditions and may from time to time substitute or amend the reward for this promotion. Phoenix Health reserves the right to cancel this promotion at any time, without notice; or extend the promotion, at their discretion.
